Description
Elvanse 70 mg hard capsules contain 70 mg of lisdexamfetamine dimesylate, which is equivalent to 20.8 mg of dexamfetamine.
Mechanism of Action: After oral administration, lisdexamfetamine is rapidly absorbed and then hydrolyzed primarily by red blood cells into dextroamphetamine and L-lysine. Dextroamphetamine, the active metabolite, works by blocking the reuptake of norepinephrine and dopamine into presynaptic neurons and increasing their release into the extraneuronal space, thereby enhancing neurotransmitter activity in the central nervous system.This mechanism is thought to be responsible for its therapeutic effects in ADHD [1]. For BED, its efficacy is also linked to enhancing cognitive control and potentially reducing impulsivity, as well as having anorexigenic effects.
Indications: Elvanse is indicated as part of a comprehensive treatment program for ADHD in children aged 6 years and over when previous methylphenidate treatment was inadequate, and in adults with pre-existing childhood ADHD symptoms [1]. It is also approved for the treatment of moderate-to-severe binge eating disorder in adults.
Dosage and Administration: The starting dose for Elvanse is typically 30 mg once daily in the morning, though a lower initial dose of 20 mg may be appropriate [1]. The dose can be increased by 10 or 20 mg increments at approximately weekly intervals, with a maximum recommended dose of 70 mg/day [1]. Elvanse can be taken with or without food. Capsules can be swallowed whole, or their contents can be mixed with soft food (like yogurt) or liquid (water or orange juice) and consumed immediately.
Side Effects: Common side effects include decreased appetite, insomnia, dry mouth, headache, upper abdominal pain, and weight loss. Serious but rare side effects can include mania, sudden cardiac death in individuals with underlying heart problems, stimulant psychosis, and serotonin syndrome. Lisdexamfetamine has a high potential for abuse, misuse, and dependence.
Contraindications: Elvanse is contraindicated in individuals with hypersensitivity to sympathomimetic amines or any excipients, concomitant use of monoamine oxidase inhibitors (MAOIs) or within 14 days of MAOI treatment, hyperthyroidism, agitated states, symptomatic cardiovascular disease, advanced arteriosclerosis, moderate to severe hypertension, and glaucoma.
Storage: Lisdexamfetamine should be stored at room temperature, between 20°C to 25°C (68°F to 77°F), in a tightly closed container, protected from light, and kept out of reach of children.
